Borough of Chatham Board of Health

The Borough of Chatham Board of Health is an autonomous public health agency created under the authority of the State of New Jersey. The Board consists of seven members, appointed by the Mayor, for a term of three years. Regular meetings are conducted on the 2nd Thursday of each month, (except for July and August) at 8:00 PM in the Municipal Building, middle level. These meetings are open to the public.

The Chatham Board of Health is under the auspices of the Madison Board of Health and employs the services of their Health Officer, Health Educator, Administrative Nurse and Sanitarians. The citizens of Chatham Borough are entitled to use any of the services offered at the Madison Health Offices, 28 Walnut Street, Madison, New Jersey. This includes periodic testing for cancers, blood testing, and maternal & childcare. Articles in local newspapers alert residents as to dates and times. Madison Board of Health may be reached at 973-593-3079.

The Board of Health employs Susan Caljean as the Administrative Secretary and William R. Faitoute, the Registered Environmental Health Specialist. Mrs. Caljean receives the initial calls and, as needed, refers out to the proper authority. This includes all calls that would involve the Board of Health. Betty Rhodes is recording secretary and attends monthly meetings.

Mr. Faitoute works closely with the Board, investigating these complaints and checking environmental issues that are reported.

The Board runs an annual flu clinic, generally conducted in late October and held for seniors at the St. Patrick Family Center. They also conduct the annual rabies clinic for dogs & cats, held in January, at the rear of the firehouse. There are no charges for these clinics. Chatham Borough officials perform marriage ceremonies if arrangements are made in advance. The Mayor may be contacted at 973-635-0674 x 213 and the Municipal Court Judge at 973-635-0674 x 206. You may also contact the Morris County Clerk at 973-285-6120 and arrange for a ceremony at the Morris County Courthouse

 

Certified copies of Birth Certificates are issued only for births that have taken place in Chatham Borough. If a Chatham Borough resident gives birth in a local hospital the certified copies must be obtained in the municipality where the hospital is located.

Certified copies of Death Certificates are issued by this office only for deaths that take place in Chatham Borough. In the event an original Death Certificate was obtained from this office by our local funeral director, any additional certificates must be obtained in the municipality where the death occurred.

Individuals requesting a record of Birth, Death or Marriage must provide exact information on a form provided by the Borough of Chatham. The relationship between the requestor and the person named on the form provided will be the determining factor in deciding whether the Registrar issues a Certified Copy of the record.

· Exact names as recorded on record.

· Exact place where event occurred.

· Exact date of the event. – Multiple years will not be searched.

· Birth – Exact Mother’s Maiden Name/Exact Fathers Name.

· Death – Exact Name

In addition to the above information you must provide a Photo ID with an address or one (1) additional form of ID with address or two (2) alternate forms of ID such as Non-Photo Drivers License, Vehicle Registration Insurance Card, Voter Registration, Passport, Green Card, County ID, School ID, or Utility Bill.

The cost of certified copies of any records is $10.00 (effective July 1st 2005). The application must be filled out and returned with the require identification. Cash, Checks or Money Orders made payable to the Borough of Chatham are acceptable.

FOOD OR VENDOR LICENSES

Individuals or partnerships that operate any type of business that serves or sells food in Chatham Borough on a permanent or temporary basis must obtain a Food License from the Chatham Borough Board of Health. Business owners must then undergo an inspection by the Chatham Borough Sanitarian.
